I use a gophone for work and I am eligible for reimbursement. To qualify, I have to present a copy of my bill with the list of charges and my cell phone number.
At one time I could look up account history for adjustments and refills.
A nice screen would pop-up that said Account History, it showed my wireless phone number, the date and time of the adjustment and the amount. I could print out that page as a pdf and send it to the company to get reimbursed.
Now when I do a lookup, it displays the charge with date and time, but no indication of the phone number. When I attempt to do a print, it is even worse. Everything is out of alignment and there is no at&t logo. Below are the before and after:
